Will the Thule TH859XT fit my 2018 Audi Q7 with the factory mounted rails and Audi cross bars?

Most likely yes, but since Thule does not test fit their cargo basket part # TH859XT on vehicles you'll have to take a couple quick measurements to know for certain. The crossbars can be spaced between 20" and 37" apart when measured center-to-center to fit.
Yakima on the other hand does test fit their roof rack accessories on vehicles and has confirmed the fit of their LoadWarrior part # Y07070 on the factory bars of your 2018 Audi Q7 so if you are looking for a similar basket that you know will fit without requiring any measuring this definitely will work for you.
