Yes, the brake controller adapter is included with the brake controller if you go through the Fitguide on the website with the 2001 Chevrolet Silverado and click on brake controller. The Hayes brake controller is part # HA81792BB. The brake controller adapter part # HA81781-HBC plugs into the junction box under the dash of the Chevy and the other end plugs into the Hayes brake controller. If your truck has a 7-way the Hayes brake controller and adapter are all you need.

If your Chevy doesn't have a 7-way plug but has a round plug at the rear of the truck with a cap on it, you can use Hopkins connector part # 30955 to plug into the round connector for a 7-way. I have attached an installation video on a 2008 Chevrolet Silverado for your reference.

If there is not a round plug there will be a bundle of wires near the rear of the truck to wire in a 7-way to. You can use Pollak 7-pole part # PK11898 to wire in the 7-way. Use a circuit tester to test the functions of the wires from the back of the vehicle using part # PTW2993 and attach the corresponding wires from the Pollak 7-way found in the installation instructions.
