My wife has had multiple back surgeries and the dealer installed running boards on my 2018 Toyota Tundra is too high. Is there an additional step available to shorten the distance she had to raise her leg for?

The Nerf style side steps will often give you a slightly lower step than the factory running boards but probably not enough to make the difference you are seeking. For your 2018 Toyota Tundra, I recommend the Carr Custom-Fit Side Steps - Hoop II - Polished Aluminum - 7" Step - 1 Pair # CARR104502 which will give you a 5 inch drop.
The other option is the Bestop PowerBoard NX Motorized Running Boards - Wireless Door Sensors - LED Lights # B7563715 for CrewMax cabs or # B7563615 for Double Cab models. With these you can use the Extension Arm Kit for Bestop PowerBoard Motorized Running Boards # B7513015 to add an additional 2 inches of drop to the running boards.
