I would like to attach a bike rack and the installation drawings show the hitch tilting down with the adapter for a ball mount. I will not have a ball mount. I assume it can be tilted to come out horizontally and still be able to drill?

The trailer hitch in question that installs at an angle on your 2000 Mazda Miata is # C11755V. It is not meant to install in any other way other than what is stated in the instructions. But there is a hitch with the receiver that is horizontal and it is # C11749.
If you want to use the hitch for an accessory such as a bike rack or cargo carrier then you have to make sure the accessory is rated for use with a Class I trailer hitch. Also, with a bike rack, you are limited to 2 bikes maximum on a Class I hitch.
Curt also says you have to use stabilization strap # 18050 when using this hitch for any non-trailer load (such as a bike rack or cargo carrier) per their warranty requirements. I have linked the Class I compatible 2-bike racks and cargo carriers for you.
