The mounting holes of the Demco Hydraulic Brake Actuator w/ Drop - Drum - Zinc - 2" Ball - 8,000 lbs # DM8669112 are in the same exact locations as you mentioned. From front bolt to second bolt center to center is 3 inches. From the front bolt to the rear bolt center on center is 12-1/2 inches. This bolt-on installation requires 1/2 inch by 4-1/2 inch Grade 5 bolts that are not included. I've attached a photo to help.
