Will the 640 Mojave heater fit my 2012 Polaris midsize ranger 500 efi sidexside and does it come with all necessary installation hardware needed?

The Flex-a-lite Mojave Vehicle Heater # FLX640 is designed for UTVs and smaller applications like what you have but it was not custom fit for your 2012 Polaris Ranger 500. It is possible for you to install this and have it work well but it would take a bit of creativity as it's not a direct fit.
