The two Westin Sportsman Grille Guards that fit the 2004 Chevrolet Silverado 3500 are shown on the linked page. Of these two your photo most closely resembles their part # 45-1240. The replacement bracket and hardware kit for this grille guard is part # 40-124PK. Please note that both grille guards and installation kits are vehicle specific so unless your grille guard is the correct one specified for your truck even the correct install kit will not allow it to mount. Both the guard and install kit must match your vehicle.
