What the diagram is trying to show is that the bolt and spacer block get routed through one access hole and the bolt will stick out of the adjacent hole with the block keeping it in place. So the block and bolt head are inside the bumper with the threaded part of the bolt sticking out.

I do have a solution for you but the reason you are not seeing a base plate for your 2019 Fiat 500L is because the owners manual for your 2019 Fiat 500L says that it can not be flat towed. According to the owners manual you will have to use a tow dolly with the front wheels of your Fiat 500L off of the ground.

Curt trailer hitch # C11364 was specifically designed to fit the 2014 Fiat 500L. The review suggests that there is no fishwire included. One of our other experts checked this and found no fishwire was included so # 80101-1 would be needed as well.
