Which black diamond plate bumper and hardware kit would I need to order to fit the rear of a 2004 Chevy Silverado 2500 with a Duramax?

We do have bumpers that fit your truck, although I checked with Westin and there is not a mounting kit for your 2004 Chevy Silverado 2500 that will allow you to install one of the universal black diamond plate bumpers.
For a similar bumper which is a custom fit if you have an '04 Silverado 2500LD there is the Westin Perfect Match Series Rear Step Bumper - Chrome Plated Steel # 31006.
