When you mention 5inch offset is that from center of rim to lug bolt mounting face?

Wheel offset is always measured from the center line of the rim to the hub face. In the case of Aluminum HWT Star Mag Trailer Wheel with +5 mm Offset # AM22691 the rim provides 5-inches of positive offset, which moves the tire surface closer to the vehicle.
