We don't have specific information on what pin box is currently on your 2004 Monaco 5th wheel, so you will need to check the spec sticker on the pin box to find the model number. If you can't locate the model number on your existing pin box you will need to take some measurements; I've linked a helpful article that explains how to measure to determine what model pin box you've got and which replacements would work for you.

For reference, the MORryde Cushioned 5th Wheel Pin Box for Up to 11.5K Trailers w/ Fabex 765 # MR22XR is designed to fit on trailers that have a Fabex 765 pin box, but the GVWR rating is only 11,500-lbs so it's unlikely that this would be the correct one. Since you've got a PullRite sliding hitch you will need to make sure your replacement has a fixed turret and not one that rotates.

I've been told that I have a Venture Medium pin box. What is the replacement for that part and also the capture plate for a Pullrite hitch?

The pinbox replacement for a Venture Medium pin box is the MORryde Cushioned 5th Wheel Pin Box for 14K to 15K Trailers w/ Venture Medium # MR95VR. You'll also need the MORryde Pin Box Adapter for PullRite SuperGlide Hitch # MR57ZR for your Pullrite hitch.
