I am looking to get Yakima HoldUp for my Jeep Wrangler JLU 2019. I am wondering if this fits with the spare tire on without the extension.

The answer really depends on the trailer hitch you have on your 2019 Jeep Wrangler Unlimited, so you may want to measure the distance from the center of the hitch pin hole to the edge of your spare tire to find out for sure.
The Yakima HoldUp 2-Bike Rack # Y02443 measures 12" from the center of the hitch pin hole to the closest wheel holder. So for example, if you had the Draw-Tite Trailer Hitch # DT78MR, it is 5-1/2" from the center of the hitch pin hole to the edge of the spare tire so it would clear easily.
However, that measurement of the etrailer Trailer Hitch Receiver # e98856 is 8-1/2" and for the Curt Trailer Hitch Receiver # 13432 the measurement is 11" so it would be pretty close.
