Yes, the Yakima HoldUp 2 Bike Rack # Y02443 will fit a 2003 Toyota RAV4 with the rear spare tire.
When we test fit the Curt Trailer Hitch Receiver # C13524 on an '03 RAV4 we found that the distance from the center of the hitch pin hole to the edge of the spare tire was 10 inches. Since the distance from the center of the hitch pin hole to the closest wheel holder on the HoldUp is 12 inches you will have room to use this hitch and bike rack together.
