Making a custom ramp like this takes some unusual parts, but I think I can help. For the wheels, I recommend the Ultra-Fab Hitch Mounted Skid Wheels for RVs up to 30' Long - 4" Diameter - Qty 2 # UF48-979015.
For the 2 speed manual crank jack, I recommend the Square 2-Speed Jack - Drop Leg w/ Spring Return - Sidewind - 12-3/8" Travel - 10K # TJD2SP-12000SP-F.
I don't know what you mean by tow boot. I assume you want a combo bar like # E-9 or a receiver like # UF35-946401.
For safety chains, I recommend the Laclede chain with clevis slip hook # 1483-535-04.
These parts will help you build a similar yard ramp system.
