I checked with Thule and they assured me that the wheel tray of the Thule Elite Van XT 2 Bike Rack for Sprinter Rear Door # TH302043 would clear your rear spare as the wheel tray is actually located above the bottom of the window edge so it wouldn't come close to interfering with your rear mounted spare.

In order to remove these clips you would need to undo the allen head bolt which can be done from outside the vehicle yes but you would then need to open the door in order to release the clip. But it is carried high enough that you could carry two of your non-fat bikes on it and then use a hitch mounted rack like the Thule T2 Pro part # TH9034XT like you mentioned for the fat bike(s) as needed.
