Question:
Should the hitch ball be 90 degrees or perpendicular to the bumper of the towing vehicle?

Weight distribution heads are adjustable to get the spring bars level or below level of the trailer frame. Depending on the trailer frame, the head may need to be set at 90 degrees, it also may need to be tilted down as far as possible to keep the trailer level. This will also depend on the weight of the trailer and strength of the suspension on the vehicle. With too much spring bar tension the rear of the vehicle could lift. Not enough tension and it will still sag.
I have added a link to a video of a weight distribution system being setup on a Dodge Durango that will show you how to adjust everything. Also, the instructions for the WD kit will tell you where to start and how to adjust once you have the kit installed.
