A regular cab short bed Sierra is about 17-1/2 feet long, and the cable included with the camera is 19-1/2 feet long. Your crew cab is a bit longer. I'd say you're right on the borderline of needing the extension. The easiest way to find our for sure if you need it would be to take a piece of string and run it from the back bumper at the trailer connector, under the car, into the engine compartment and through the firewall into the passenger cabin to where you're wanting the monitor to mount. When you route the string underneath, be sure to avoid anything hot or moving, like exhaust and steering or suspension components. Then, measure the length of the string. If it's longer than the 19-1/2 inch long cable included with the camera kit, you'll need the # HM3100504060 5 foot extension cable.
To route through the firewall, look for an existing grommet that wires pass through. You can poke a hole in the grommet, and pass the wire through it.
