Sounds like you have either the LC736514-ELKIT or LC736515-ELKIT. In that case, you will want the Replacement Remote Control for Wireless HappiJac Electric Truck Camper Jack Systems - 25 Amp # LC723915. If your model number is different, please let me know.
The # LC723915 does not include the cord, so if you need that as well, be sure to grab the # LC149557.
