Judging by the provided photos, you have the Thule Xsporter Pro Adjustable Ladder Rack # TH500XT.
For the mounting plates, you will need the Replacement Base Assembly for Thule Xporter Pro Ladder Rack - Right Front or Left Rear Assembly #TH7521367001 and the Replacement Base Assembly for Thule Xporter Pro Ladder Rack - Left Front or Right Rear Assembly # TH7521368001.
The part that you need for the endcap is the Replacement End Cap for Thule Xsporter Pro Truck Bed Ladder Rack # 852-3214001.
