The closest inner tube gooseneck coupler to the 4" x 30" dimensions you mentioned is the Bulldog BX1 Gooseneck Coupler Inner Tube - Round - 2-5/16" Ball - 25,000 lbs # BD1289020300 which has a 4" diameter and is 28-1/2" tall. This coupler has a 25000lb weight rating, and when used with the outer tube has 8" of vertical adjustment.

If you need a coupler with a higher weight rating I recommend Convert-A-Ball Multi-Cushioned Gooseneck Trailer Coupler - Round - 30,000 lbs # CG which has an over height of 25.94". The # CG does not require support pins so you be able to adjust it to the exact height needed.
