Where do I run the wires from the vehicle to the connector and which color wires go to the brake lights, the tail lights and the running lights?

Actually installing a 4-way trailer connector will be a little more involved than just wire a 4-way to the tail light wiring of your 2012 Nissan NV. You will actually want to purchase and install a Tow Ready Custom Fit Vehicle Wiring # 118553. This comes with everything you will need to provide you with a fully functional 4-way at the rear of your vehicle.
You will need to run a wire from the converter box to the positive side of the battery also. I attached installation instructions for this product for you to check out also.
The 4-Pole # 18004 that you referenced would be good for a replacement 4-way that has already been wired to a vehicle and you just wanted to replace just a damaged connector.
