Can this hitch work with a box or do I have to have a flatbed and if so do I need to make a hole in my box?

Yes, the Curt Overbed, folding Ball Gooseneck Trailer Hitch, # C16321-52, will work in the bed of a your 2006 Ford F-150.
This hitch will bolt on to the frame of your truck and will not require any welding to install. The Curt hitch has a 30,000 lbs gross towing capacity and a 6,000 lbs vertical load limit.
