A trailer hitch cover can be held into a hitch receiver in a couple of ways. First, some have a built-in device that is sort of like a wishbone. On those you would just pull out the cover. Second they can be held in with a hitch pin and clip or hitch lock. For a regular hitch pin you would pull off the clip, remove the pin, and pull out the hitch over. On a lock you would unlock it and pull out the pin (some lock have to be unscrewed).
If there isn't enough of the hitch cover left to grab a hold of you should still be able to grab it with some needle nose pliers or push it out a little through the hitch pin holes to be able to get a grip on it and pull it out.
Then you could replace it with a new cover. I have included a link to the GMC hitch covers we carry. A metal one such as # AUT-GMC2-C or # AUT2-GMC2-C will last longer than a plastic one.
