A ball mount like the 180 Hitch 2-Ball Mount w/ Stainless Steel Balls # LTB4-2 will allow you to use the hitch ball rise position with the extender unless there is a noted restriction on your extension. Most extenders do not have that restriction.
For instance, the Curt Hitch Extender for 2" Trailer Hitches - 14" Long # C45795 has restrictions of a maximum tongue weight of 350 lbs and maximum towing of 3,500 lbs. You will also want to recognize that the use of an extension will reduce the hitch's weight capacity of 50%. So, if your hitch is rated for 500 lbs tongue weight, the tongue weight you can use with your extension is only 250 lbs. The use of the rise position does not change this capacity even though it will add more torque to your hitch...that is accounted for in the restrictions already given.
