Question:
Im getting 12VDC at all outputs of the module all the time! Module is 5 years old not much usage testing done with no trailer attached. Is this a bad module?

We offer three brands of 4-pole wiring for your car, all shown on the linked page, and the one I suggest based on reliability is the Tekonsha # 118482. An install on a Venza is shown in the linked video.
If your Venza's 4-pole trailer harness shows 12V output on all three circuits all the time then the module has failed. The output circuits should have voltage only when the stop/turn/tail light functions are activated on the car.
