    That is a pretty tall trailer frame but we do have a solution Dan. You could use the part # HD50000101 unless you wanted a swivel option. Then I would go with a weld on style jack like the etrailer # TJD-7000S for a sidewind or the part # TJD-8000 for a top wind. Or if you wanted something that could swivel you'd want a pipe mount like the part # BD178203.

    I believe you're referring to is the Pro Series Round, Pipe-Mount Swivel Jack # PS1401440303, and the pipe this mounts too cannot be bolted on, it must be welded on.     I went out into our warehouse to get the dimensions of the # HD50000101 you requested. I attached a couple pictures that detail all of the dimensions. Basically the holes are 1/2 inch in diameter, the closer holes are 1 inch apart on center and the bigger distance between the upper and lower holes are 2-5/8 inches, and the distance from left to right is 4 inches.

Instead of trying to match the bolt pattern of the welded plate you can get a bolt-on jack that uses U-bolts to connect to the trailer.
