The Curt Above-Bed Gooseneck Trailer Hitch for Fifth Wheel Rails # 16055 can be used with a set of in bed rails. This will give you a 3" offset.

The other option would involve putting an offset adapter on to your gooseneck trailer. Something like the B&W Extend-A-Goose Gooseneck Coupler # BWTEXA4200. If you decide that this is the best option for you let me know what tube size you have on your gooseneck trailer and I will recommend a option for you.

Yes, the B&W Turnoverball Underbed Gooseneck Trailer Hitch # BWGNRK1320 will fit on your 2021 Ram 3500. You will have to remove the center gooseneck ball assembly that is currently on your Ram. It is independent of the outer puck mounts so that you will not have to remove those mounts. This will allow you to use the B&W 4" Offset Ball # BWGNXA4085 with your truck.
