The Yakima Big Horn bike rack you mention, part # Y02404 provides 11-1/8 inches of clearance between the hitch pin hole and the closest portion of the rack, which is the upright.

To determine if it will work on your Town and Country, you will need to measure rearward from your receiver hitch pin hole to the furthest rearward portion of the bumper, as shown in the photo I edited for you. If this distance is less than the 11-1/8 inch clearance provided by the rack, it will fit on your Town and Country.

We have found that the bumper clearance is typically around 3 to 5 inches, depending on whether your Town and Country is a stow-n-go model or not. The rack would most likely fit, but I would still recommend you measure it for yourself to confirm.
