Will this fit my gmc with a over the rail bed liner? If so are there any modifications?

The Access Tonneau Cover, # A22020219, will not fit with an over the rail drop in bed liner. You would need to remove the portion that goes over the rail (making it and under the rail bed liner) to get a tonneau cover to fit.
The best option for a 2003 GMC Sierra with a 6-1/2 foot bed is the Craftec cover, # 251929. You will just need to trim the over the rail bed liner where the unit clamps to the truck as opposed to trimming large portions of it.
