In order to have a 5 pin flat trailer connection you will first need to install the Tow Ready Custom Fit Vehicle Wiring, part # 118381, that you referenced. I attached installation instructions and a link to an installation video for this product below for you to check out also.
The next thing you will need is an Adapter 4 Pole to 5 Pole Trailer Wiring Adapter, part # 20036. This adapter will plug into the 4-way that you just installed with the # 118381, and then you can run the blue wire to the reverse light circuit behind one of the vehicle tail lights to activate your electric lockout for your surge brakes.
