On my vehical the liter does not work unless the key is on. Will this item still work? or does the key have to be left on ?

The CTEK Comfort Connect Cigarette Plug Charger Cable, part # CTEK56263 will not work to charge the vehicle battery unless the circuit feeding the cigarette lighter is live.
Many vehicle have an additional power outlet that is powered when the ignition is switched off. If this is the case with your 2003 Toyota Tacoma, you can use the cable with that outlet.
Leaving the key on would not be a good solution, as the power drain from having the ignition switch on would negate any charge received by the battery.
